A good quality carpet underlay should last as long as your carpet — typically between 10 and 25 years, depending on the product, the level of foot traffic and the quality of the installation.
The key word here is quality. A budget underlay may compress and flatten within just a few years, losing the cushioning and support properties that make underlay worth having in the first place. Once an underlay has flattened, it's lost much of its thermal and acoustic performance too, and it stops protecting your carpet from below.

A note on reusing underlay
When you replace your carpet, it can be tempting to reuse the old underlay. In some cases, if the underlay is in genuinely good condition and less than a a couple years old, this may be acceptable. But if the underlay is compressed, stained or simply old, replacing it at the same time as your carpet is always wise.
